Physical Exam for Participation

Physical Exams are required for a child to participate in athletics.

Physical Exam forms must be turned in BEFORE a child can participate (this includes tryouts).

Take the Physical Exam form with you to your primary health care provider for a physical.

Physicals are good for one year and can be used for multiple sports.

Important Dates:
August 16 - Informational Meeting for students in room A-2 from 3:15-3:45
August 18-19 - Open Gym (3:15-5:00 PM in Anderson Gym)
August 22-23 - Tryouts (3:15-5:00 PM in Anderson Gym)
September 7 - First Match (4:30 - 6:30 Anderson Gym)

Open gym and tryouts will take place during the first few weeks of school so please schedule your physicals soon if you haven’t already done so. You must have your APS physical form filled out and signed by your doctor to be able to participate in tryouts. Print out the form from the link below and take it to your appointment with you. Many primary care physicians, urgent care locations, and mini-clinics offer shorter athletic physicals to complete the physical and physical forms for you.

Required Paperwork for Tryouts:
The following forms MUST be completed and turned in for a student to be allowed to participate in tryouts. Please make sure that ALL required forms are included and signed on the appropriate lines. They cannot be turned in after tryouts; no exceptions.

2ND REQUIREMENT - CONCUSSION FOR STUDENTS COURSE CERTIFICATE
Concussion for Students Course:
STEP 1 - Concussion For Students website
STEP 2 - Choose “New Mexico” on the “Select Your State” pulldown menu and click “Order Course”
STEP 3 - Click on “Register” to create an account (use student’s name when registering - this is the name that will print on the certificate)
STEP 4 - Click “Checkout” to complete your order (the course is free)
STEP 5 - Complete the course and print the completion certificate and bring to tryouts
This course must be completed annually.
Email Coach Antillon (annamarie.antillon@aps.edu) your certificate if you do not have access to a printer.
3RD REQUIREMENT - APS ATHLETIC AND ACTIVITY CODE OF CONDUCT
Print and fill out the Athletic and Activity Code of Conduct Acknowledgement form
4th REQUIREMENT - GRADE CHECK

Spring 2022 Semester Grades - you can find these on your StudentVue account. If a student received a D or an F in any class, they cannot participate in tryouts. (6th graders are not required to turn in a copy of their grades)

Once a player is on the team, they will be required to keep their grades up and will not be allowed to practice or participate in matches if they have a D or an F in any class. If a student is not able to raise their grades after a two-week period of time, they will be removed from the team.
